• Chronic Kidney Disease (CKD) is a progressive chronic disease. In 2023 a TILDA report showed that
more than 1 in 7 people aged over 50 years in Ireland have CKD and this number continues to rise 1.• Service users with CKD require individualised and detailed education regarding their disease
condition, treatment options and dietary management.• At the Midland Regional Hospital Tullamore (MRHT), service users attending the Nephrologist are
referred to the Joint Renal ANP And Dietetic Led Low Clearance Clinic (LCC) when their kidney
function reaches <20ml/min/1.73m2.• In order to assess our service user’s overall experience of attending the LCC, their knowledge and
understanding around CKD, we conducted a survey, and evaluated overall experience, interaction
and the quality of intervention provided during their visit to the LCC.• The results of the survey indicated the need for a virtual app to ensure service users are provided
with timely, individualised and clinically appropriate information to support their CKD journey, to
promote their involvement and empowerment in their care and to improve clinicians work efficiencies.
